How to Register Your Child for Completely KIDS

- Review the Parent and Student Handbook. Available in English and Spanish, you may download them here.
- Review and complete the Program Information and Registration Form. Submission of this form does not guarantee your child a spot in our after-school program. Upon submission of the form, you will be contacted by a site coordinator. You may complete the form online via the link here or by downloading a pdf. Please complete one form per child.
- Complete the Child Care Subsidy form online at the following link. Child Care Subsidy (previously known as Title XX) is part of the Social Security Act and provides funding for social services through the SS Block Grant. States receive the money and can decide how to spend the money. In Nebraska, some of the funding is used to help parents afford child care. The state pays day care providers directly for the time the children spend in day care. For questions about Child Care Subsidy enrollment, contact your school’s site coordinator. If you apply for this subsidy and are denied, you may apply for a Child Care Subsidy Scholarship. Proof of denial including the reason for denial must be included with this application.
- Complete the Release of Information (ROI) form available from the Completely KIDS coordinator at your child’s school.
